RCI Honors Georgia's Caregivers of the Year Care Transition Coach Twenty-five individuals have been trained to serve as Care Transition Coaches at a recent training in Athens, Georgia. Training was led by Dr. Eric Coleman and colleagues from the University of Colorado Denver. The Care Transitions Intervention is an Evidence-Based program designed to empower patients and caregivers to effectively manage transitions between care setting. The training was sponsored by RCI and CARE-NETS as part of the Georgia Coalition for Caregiver Health. Read more about Care Transitions. Georgia Coalition for Caregiver Health Rosalynn Carter Institute and its partner CARE-NETs have launched a new state-wide Advocacy organization dedicated to promoting caregiver health in Georgia. The Coalition will work to establish state-wide training to equip caregivers with the skills they need to manage their health, deal with high levels of stress, and solve caregiving challenges. The Coalition will also work to assure that all caregivers have access to affordable health care and health insurance. Find our more: Donate to RCI
