The October session of the Telephone Learning Series is titled: "Care Transitions: Handling Changes in Care Settings and Care Providers." It will be held Thursday, October 18 at 12 noon EST.
The term “care transitions” refers to the movement patients make between health care practitioners and settings as their condition and care needs change during the course of a chronic or acute illness.
Care transitions are especially vulnerable times for patients, often characterized by conflicting medical advice, medication errors and a lack of follow-up care. These factors diminish the quality of care, thus necessitating additional treatments that might have been avoided. Dr. Coleman developed an intervention designed to target these problems and ease the transition between care sites and health care providers.

The Patient Health Record that consists of the essential care elements for facilitating productive communication during the care transition.
The Discharge Preparation Checklist of critical activities designed to empower patients by ensuring they have the information needed to provide effective and comprehensive care.
The Medication Discrepancy Tool is a tool for identifying and characterizing medication discrepancies that arise when patients are making the transition between sites of care.
